To refer a patient, please prepare the Patient History, Physical Records, Patient’s Medical Records (History and Physical; Medication List) and Doctor’s Orders, then fax them to us at 909-839-2529. You can also connect with our Business Development Team for further assistance.

To be eligible for home health care, a patient must be under a physician’s care, homebound, and require intermittent skilled services. Care plans include hands-on treatment and medical guidance as prescribed by your doctor. These services are designed to promote healing, safety, and independence at home.

Advent Healthcare Services, Inc. primarily in an appropriate service based on the physician’s request. Within 24 to 48 hours an Advent Healthcare representative visits the patient unless otherwise specified.

If you are homebound and unable to visit a clinic, we can help connect you with trusted mobile physicians who can visit you at home for an assessment. As long as you meet insurance eligibility requirements, they can provide the necessary doctor’s order for home health services.
